The
functions of the Faith Community Nurse.
- 1. Health Educator
- Educate individuals and small groups in the community on lifestyle
factors, faith and relationships, health enhancement activities, illness
risk reduction, disease management, environmental awareness, social justice
issues, any other health and well being issues which are pertinent to the
group of people the faith community nurse serves.

- 2. Personal Health Counsellor
- Personal and small group counselling and advocacy. Helping people with
health, problems, listening, advising as required, supporting, recommending
referral if required, home visits, monitoring progress.

- 3. Referral and liaison
- Negotiate access and assist entry into the support networks which people
may be unaware of. Support structures include; pastoral support, family
resources, congregational resources, food kitchens, self-help groups,
support groups, government agencies, legal aid, financial planners, home
care, shelters, nursing homes, health and welfare professionals.

- 4. Coordination of volunteers
- Train groups of ancillary workers and volunteers to; facilitate
congregational support, help individuals, families and groups, provide
regular visiting, provide regular physical support.

- 5. Care management services
- Assist clients with existing illnes, complex conditions and/or disability
to manage their condition and prevent or limit complications.

- 6. Nurturing spiritual growth
- Help people to integrate their spiritual dimension so that even in the
presence of incurable disease, people can find well-being. All health
education is aimed to assist people in the process of becoming, to draw
closer to God thus nurturing their growth toward wholeness through Jesus
Christ.
