Surgery may be considered as the disease progresses and the drugs become less affective. There are several different procedures.

A pallidotomy has the effect of reducing (even eliminating) the dyskinetic movements associated with long exposure to, or high doses of, levodopa. After the surgery levodopa can be used at higher doses.
