
This program is a Home and Community Care (HACC) Program developed to prevent isolation and provide social links for older people as well as younger people with disabilities and their carers who are unable to access social activities or appointments without assistance and often do not have other support networks in place.
Support is provided to people who are housebound or isolated due to mobility difficulties, lack of confidence, geographic or other factors. The goal of Social Support Services "is to contribute to maintaining and enhancing the security, independence and emotional, social and intellectual well-being of people from the HACC target group who are, or at risk of becoming, socially isolated ". Social support is offered to older people through transport assistance to allied health services, day care centres, etc.
Assistance also includes library book delivery, theatre groups, lunch and afternoon tea groups, community shopping days, special interest groups, and special occasion outings. For more information on registering as a client in this program please contact the MVRC social support worker on: 9562 0414
